Reporting an alert that did not fire
The useful details are the character name, roughly when the deadline was, and which of the four alert types it was. That is enough to find the poll in the logs. A screenshot of a notification that never arrived is, understandably, harder to provide.

An EVE mail to Sakura Tamashi reaches me as well. It is slower than the form, because it depends on me logging in, but it is the option that costs you nothing: no email address, no name, nothing stored on this side at all.
